# **Part 2 - Business Background Questionnaire**

Thank You and Congratulations for completing Part 1 of our 2 Part Questionnaire. The good news is that this is the final piece of information we need from you to proceed to the next stage of our journey.

*PS...... If you have come here without completing Part 1, don't worry, just let your coach know and they will send you on Part 1 to complete later.....*

Please complete this questionnaire as soon as possible. This questionnaire is designed to accomplish 2 things:

 to help me determine if there is a good fit between you and your business goals and my coaching services

 to help make our initial coaching session as productive as possible

 

This questionnaire includes general questions about your goals and ownership/leadership style as well as more specific questions relating to your business. It is important for you to be as honest and accurate as you can.
